At the ASHP Midyear 2017 meeting, Amy Westmoreland, pharmacy manager and clinical pharmacist at Carilion Giles Community Hospital took some time with Pharmacy Times to discuss the importance of collaboration between hospital pharmacists and physicians. Westmoreland was recognized at the Pharmacy Leadership Institute Annual Alumni Breakfast held during the meeting. Pharmacists need to be able to communicate with physicians when medications need to be optimized or stopped, particularly in elderly patients, where polypharmacy is a common issue.
