"Heart failure is associated with almost one million hospitalizations annually. Preventing readmissions through evidenced-based medicine is a large part of managing this patient population. As a result, much of their care falls to community physicians.

I am excited to introduce an upcoming Peer Exchange focusing on congestive heart failure management. The topics will include: disease state information, understanding the current clinical guidelines, and applying them to hospital discharge planning to ensure a smooth transition of care to the community setting. Current therapeutic options, including the clinical rationale for treatment decision making will also be discussed.

The esteemed panel will provide the valuable perspectives of the cardiologist as well as the clinical pharmacist in managing this disease.

This video series will become available early 2017. We hope you will find it useful and informative."
