Khanh Pham, pharmacy manager, and Katy McGroarty, senior certified pharmacy technician, were baffled when this prescription came into their Walgreens pharmacy in Leas Vegas, Nevada. The medication written was stumping them, and several phone calls were required to clear up the confusion.

This prescription made the rounds at CarePlus Pharmacy in Columbia, South Carolina. After 4 pharmacists and tech Amanda Lowe, CPhT, could not figure it out, a nurse at the prescriber’s office was finally able to clarify.

Rx 2: Lasix 40 mg, # 60, 1 pill alternating 1 and ½ pill
