We ask our readers to send us those eye-straining, baffling prescriptions that they receive for inclusion in this monthly feature. If you would like to contribute to this column, send a clean photocopy of the script along with a note that describes the experience and provides the correct drug information to: Can You Read These Rxs?, Pharmacy Times, 241 Forsgate Drive, Jamesburg, NJ 08831, or fax it to 732-656-9267.
Rx 1: This prescription was mind-boggling. After 2 pharmacists took a crack at trying to decode it for about 4 hours to no avail, Registered Pharmacist Domenick Mineo, of Warbasse Pharmacy in Brooklyn, NY, stepped in and placed a call to the prescribing physician for clarification. Can you unscramble this prescription?
Rx 2: Registered Pharmacist John R. Cully, of Newman Medical Center Pharmacy in Shattuck, Okla, received this prescription on a Friday and was unable to read it. He called the prescribing physician's office, and it was closed for the weekend. Finally, on Monday after 3 calls from Cully and 2 from the patient, someone at the physician's office called back and confirmed the order. Can you translate this prescription?
